Consent to Collect and Process Personal Data

By checking "I consent to the processing of my personal data" on the booking form at status-dental-center.kz, I give my consent to Status Medical Clinic LLP (BIN 251140001311, Almaty, 113 Timiryazev St), referred to below as "the clinic," to collect and process my personal data on the following terms.

1. What data

The name, phone number and selected service I enter in the form, as well as any data I share with the clinic by phone, in messaging apps, or by email.

2. What for

To contact me, book my appointment, answer my questions about the clinic’s services, and send appointment reminders.

3. What actions with the data

Collection, recording, storage, use, clarification, sending back to me in response, and anonymization. To contact me, the clinic may use phone calls and messaging apps (WhatsApp, Telegram).

4. Duration

This consent is valid until the purposes of processing are achieved, or until I withdraw it.

5. How to withdraw consent

I can withdraw my consent by writing to [email protected] or calling +7 776 035 65 33. After withdrawal, the clinic stops processing my data and deletes it, unless the law requires it to be kept longer.

6. Confirmation

I confirm that the data I provided is accurate, that I have read the Privacy Policy, and that I understand I should not share information about my health through the site before an in-person visit.
